Noise Monitoring
Have noise surveys been performed? (Sound level meters and/or dosimeters)
Are there adequate samples of employee exposure?
Have all employees been notified about the noise monitoring results?
Was all the equipment used in the monitoring calibrated properly?

Audiometric Program
Are baseline and annual hearing tests performed? Baseline within 6 months
Did the employees avoid noise before the test or wear hearing protection?
Was the hearing test performed in a quiet area meeting ambient noise specifications?
Was the person performing the test qualified?
Were employees notified and referrals done?

Employee Training
Was initial and annual training performed?
Did the training contain proper content?
Do the employees understand and demonstrate knowledge?

Hearing Protection
Are there a variety of ear plugs and muffs available at no cost to the employee?
Are the hearing protectors worn correctly and when required?
Were the employees given training on the correct way to wear them?

Administrative/Engineering Controls
Is it feasible to engineer-out the noise if over 90dB?
What administrative controls have been used for noise exceeding 90dB?

Calibration
Are audiometers calibrated?
Was s functional check of the audiometer performed before the tests?
Were all sound level meters and dosimeters calibrated before use?

Documentation
Were hearing test, records, noise monitoring and related records kept in accordance with the noise and record keeping rule?
